This form is to be used by arts teachers directly employed by the Arts Educaiton Branch before scheduling professional development in LAUSD. NOTE: Refer to the Teaching Plans below when completing this form. DIRECTIONS: 1. Download the MS Word form to your computer. 2. Option 1: You may fill out the form electronically and email it to karen.coates@lausd.net. 3. Option 2: You may print out the form, manually complete it, and mail it to: Arts Education Branch 18th Floor 333 So. Beaudry Ave. Los Angeles, CA 90017 Attn: Karen Coates, K-12 Coordinator 4. Your proposal must be received no later than six weeks before the beginning date of the first proposed session. 5. Submitting this form is not a guarantee of approval. Appropriate staff members will review your proposal and respond to you within two weeks. Upon receipt of signed approval, you may go forward with the scheduled professional development. NOTE: Refer to the Teaching Plans below when completing this form.
